Missing records questioned during inquiry

Story PictureWhen the Deputy Auditor General, Wayne Simon, testified on the first two days of the Inquiry, he spoke of the issue of missing records that his department had requested in order to conduct a thorough audit. But those records were never provided so the department had to make its conclusions on how certain transactions occurred. Today, the issue of the missing records came up again and according to Shevan Fairweather, the person who is tasked to locate those records is the Stores Manager.

“Upon request of the auditors, the Stores Department could not locate for a certain period—I cant recall exactly what were the areas they were asking for. I had checked with Mr. Herrera and I had asked if he could locate the copy of the requisition and I was told by him that when he took over last year, no records were there. I can’t really understand why he would have said that because at the time when he took over, the unit manager and the assistant unit manager was still at that department.”

“Miss Smith was the assistant at the time. She was also a manager before Mr. Usher. So I can’t understand why he could not locate them or he did not want to locate them. What I did, I asked Mr. Acevedo, who was another stores clerk, if he could come and locate the documents for myself. Two weekends went by and there was a storeroom on the third floor of the hospital that he said that they were clearing out and he would have tried to locate it for me. After the two weeks passed, what I did I spoke to—we had several staff that were at the department at the time that was working there, that if Mr. Herrera wanted to know where those records were he could have inquired from them where the records were. What I did, I spoke to two of the persons that I know were there and asked them if thy could give me an idea of where the records were but that was after the auditors had left and they gave me an idea where the records were.”

Fairweather was asked to try and provide the records to the Commission by Monday.
